Reducing tuberculosis transmission in geographic "hotspots" where infections are highest could significantly reduce TB transmission on a broader scale, according to a study led by researchers at the Johns Hopkins Bloomberg School of Public Health. An analysis of data from Rio de Janeiro showed that a reduction in TB infections within three high-transmission hotspots could reduce citywide transmission by 9.8 percent over 5 years, and as much as 29 percent over 50 years. The study was published May 28 by the journal PNAS.

"Targeting treatment of 'core groups' as a way to reduce community-wide transmission is common with diseases like HIV and malaria, but is less accepted as a mantra for TB control," said David Dowdy, MD, PhD, ScM, lead author of the study and assistant professor in the Bloomberg School's Department of Epidemiology. "Our findings suggest that hotspots containing 6 percent of a city's population can be responsible for 35 percent or more of its ongoing TB transmission. Controlling TB in these hotspots may have a similar impact on long-term, community-wide TB incidence as achieving the same targets in the remaining 94 percent of the population."

For the study, Dowdy and his colleagues developed mathematical models for TB transmission using surveillance data from Rio de Janeiro. Each model tested different scenarios for TB transmission between the hotspot and the rest of the community. Co-infection with HIV was also factored into the model.

According to the study, reducing TB transmission rates in the hotspot to those in the general community reduced citywide TB incidence by a mean 2 percent per year over the first 5 years. By year 50, TB incidence was reduced by 29.7 percent, reflecting a 62.8 percent reduction in incidence in the hotspot and a 23.1 percent reduction in the remaining community.

Tuberculosis infects more than 8.8 million people worldwide, resulting in 1.4 million deaths each year. The disease is known to cluster in hotspots typically characterized by crowding, poverty and other illnesses such as HIV. Nevertheless, TB transmission appears to be more homogeneous than that of many other infectious diseases, in which 20 percent of the population may generate 80 percent of infections.

According to Dowdy, "TB may not follow the same '80/20' rule that we see in parasitic or sexually transmitted diseases, but the '35/6' rule seen in our study suggests that targeting hotspots is still the best way to control TB in a community."

"We are investigating the theory that it was industrial sabotage," the GRU officer is quoted as saying. He said that Russian intelligence has long monitored the activities of US military electronic specialists at the Jakarta airport.

"We know that they have special equipment that can cut communications between an aircraft and the ground or interfere with the parameters on board," he said. "For example, the plane is flying at one altitude, but after interference from the ground onboard equipment shows another."

The investigation has so far turned up the plane's "black box" cockpit voice recorder, which shows that no system-failure alarms went off during the plane's final minutes, nor did the crew take any audible emergency action. But the aircraft's digital flight recorder, which monitors flight systems and engine performance, remains missing in the rough jungle terrain around Mount Salak, where at least seven other deadly air disasters have occurred.

Big questions

The biggest question about the crash is why the SuperJet's pilot,?Alexander Yablontsev, one of Russia's most experienced test pilots, requested permission to descend amid a rainstorm in a notoriously mountainous area ? and why a ground controller in Jakarta granted that permission.

"Maybe he didn't see that the plane was heading straight at the mountain. On the other hand, we don't rule out the possibility that this was deliberate industrial sabotage to drive our aircraft from the market," an unnamed official at Sukhoi, the plane's manufacturer, told Komsomolskaya Pravda. "Fortunately, we don't foresee any loss of orders [for the SuperJet]."

For a start, the ruling circle of Ilham Aliyev is using the event as a giant photo-op to present the country in a positive light. Historically, Azerbaijan?s oil and gas abundance was so famed that Zoroastrians of ancient times would build religious temples on the sites of surface wells that spontaneously ignited with skyward flames. At the beginning of the 20th century, before the discovery of Arab oil, almost half of all the world?s then supply came from Azerbaijan.

This wealth was a major factor in why the Bolsheviks in the newly formed Soviet Union moved swiftly to head off a brief window for Azerbaijani independence following the collapse of the Russian Empire in 1917. During the Second World War, the German Wehrmacht targeted Azerbaijan in a bid to choke off Stalin?s staple oil supply.

Azerbaijan has become something of a thorn in Russia?s southern flank, thwarting Moscow?s interests in its volatile former Caucasus backyard of Georgia, South Ossetia and Armenia.

Azerbaijan?s relations with Iran in recent years have also suffered a marked cooling as the former aligns ever more closely with Washington. Azerbaijanis form the largest minority population group within Iran ? some 24 per cent of the total. Recent dissent within Iran among the Azeri population has underscored the potential for creating internal political problems for the government in Tehran.

Bankia seeks US$24-billion bailout as debt worries mount

The Bankia group, a Spanish lender nationalized earlier this month, will seek 19-billion euros (US$23.8-billion) of government funds as it provisions against real estate and non-property loans.

The group will ask the state?s bank rescue fund to provide the money by buying shares in its parent company, Banco Financiero y de Ahorros, it said in a filing to regulators yesterday after a meeting of the board of Bankia SA, its listed arm. The group needs a further 4 billion euros to cover real estate on top of provisions already ordered by the government, and 5.5-billion euros for the rest of its loan book, it said.

?Bankia is the tip of an iceberg as we?ve been saying all along . . . It?s systemically important and it needs to be dealt with properly?

The unraveling of Bankia has deepened concern about the health of Spain?s banks and increased the government?s financing costs as it struggles with the debt crisis. The restructuring costs for BFA, which come on top of 4.5-billion euros from a first government bailout, compare with a May 11 estimate from Economy Minister Luis de Guindos that less than 15 billion euros of public funds would be needed to support the whole industry.

?Bankia is the tip of an iceberg as we?ve been saying all along,? said Tobias Blattner, an economist at Daiwa Capital Markets in London, in a phone interview yesterday. ?It?s a very large institution, it?s systemically important and it needs to be dealt with properly.?

The Bankia group, formed in 2010 from a merger of seven savings banks led by Caja Madrid and Valencia-based Bancaja, last year listed its banking business on the stock exchange after parking its worst real estate assets in the parent company.

Bankia was among three Spanish banks that had their credit ratings cut to junk by Standard & Poor?s yesterday, citing Spain?s weakening economy. S&P lowered Bankia to BB+ from BBB- and BFA to B+ from BB-, the ratings company said in a statement. Banco Popular Espanol SA and Bankinter SA were reduced to BB+ and Banca Civica SA was cut to BB.

Bankia announced yesterday a restated 2011 loss of 2.98-billion euros, ?without reservations from auditors.? The company plans to reduce its board to 10 members from 18, with only Chairman Jose Ignacio Goirigolzarri and Chief Executive Officer Francisco Verdu remaining, it said in a filing.

Goirigolzarri will brief reporters on the restructuring plan today at 10 a.m. in Madrid and the bank will hold a webcast for analysts and investors at 12.30 p.m.

Spain said May 9 it would nationalize Bankia?s parent, making the state the biggest shareholder in the listed unit, just days after Chairman Rodrigo Rato resigned and the lender released 2011 accounts that weren?t signed by its auditor.

More Provisions

De Guindos on May 11 ordered lenders to set aside 30 billion euros to cover still-performing real estate loans, adding to demands made in February for provisions and capital of 53.8-billion euros.

Spain?s 10-year bond yield has climbed to 6.31% from 5.73% on May 7, the day Rato announced his resignation. Bankia has declined 58% in Madrid trading, to 1.57 euros, since selling shares to the public in July. The stock was suspended on May 25.

The additional 4-billion euros that the bank is seeking to cover real estate takes its total provisions for those assets to 12.7-billion euros. In addition to 5.5-billion euros to cover non-real estate lending under an ?adverse? stress scenario, the group needs 6.7-billion euros to clean up stakes in companies and tax assets, the group said.

?Big Number?

The group expects the state rescue fund, known as the FROB, to take control of BFA through the capital increase in June. After that, the Bankia unit will be recapitalized by a 12-billion-euro rights issue guaranteed by BFA that will probably go ahead in October, the group said.

The Bankia group will have a so-called principal capital ratio, a measure of financial strength, of 9.8% and a ratio of provisions to real estate assets of 48.9% after the cleanup, it said.

De Guindos told parliament on May 23 that Spain would provide as much public money as necessary for the Bankia group as he said it was a ?specific case,? without any read-across for the rest of the industry.

?It?s a big number for the restructuring, much more than the government had previously required, and shows the importance of going in and having a good look at the assets,? said Daragh Quinn, an analyst at Nomura International in Madrid.

Myanmar police move against spreading power protests

YANGON (Reuters) - Myanmar police broke up a protest against power cuts by several hundred people in the town of Pyi on Thursday and five members of Aung San Suu Kyi's opposition party were taken in for questioning, a senior party official said.

Members of Suu Kyi's National League for Democracy (NLD) party were also briefly detained for questioning in the city of Mandalay in the early hours, a second NLD official said.

Demonstrations have taken place in several towns this week, including the commercial capital, Yangon, as citizens test the limits of democratic changes in Myanmar, leaving the authorities struggling to respond.

Until now, the security forces have allowed the peaceful demonstrations to go ahead and the civilian government, which took over from a repressive junta in March last year, has promised emergency measures to increase the electricity supply.

"So far as I heard from our members in the region, there was a protest of about 400 people at least," NLD official Nyan Win said, referring to the area around Pyi, about 260 km (160 miles) northwest of Yangon.

"The police tried to disperse them and there was some rough manhandling and some people were injured. Five NLD members were picked up for questioning," he said.

Ba Shi, a member of the NLD in Pyi, said the five were being held at the town's prison and a crowd had assembled outside to demand their release.

Tint Swe, an NLD committee member in Mandalay, told Reuters that he and two other party members were picked up by special branch officers at around 5 a.m. (2230 GMT on Wednesday) and questioned about who was behind the protests.

They were treated well and taken home about five hours later, he said.

State television said on Wednesday six generators purchased from U.S. firm Caterpillar Inc would be air-freighted within a week and two 25-megawatt gas turbines would be bought from General Electric Co to tackle the power shortage.

Urgent repairs would be carried out on power stations damaged in fighting with ethnic Kachin rebels, it added.

For decades, military authorities crushed protests against their rule, which often began because of frustration over bread-and-butter issues. The biggest and bloodiest uprisings against military rule, in 1988 and 2007, were sparked by economic grievances including soaring prices.

"WAIT AND SEE"

This week's marches pose a problem for reformist President Thein Sein - himself a former junta general - who has freed hundreds of political prisoners, relaxed state censorship, started peace talks with ethnic rebels and held by-elections that put Nobel Peace Prize laureate Suu Kyi into parliament.

But the political and economic reforms are likely to raise expectations that both the government and the opposition led by Suu Kyi might struggle to meet.

Heavy-handed tactics to end the protests would be seized upon by government critics skeptical about the reforms, while the authorities will worry the demonstrations could spread.

The disturbances come as Suu Kyi is planning her first foreign trip in 24 years with a visit to neighboring Thailand next week for an economic conference.

Suu Kyi spent 15 years in detention under the junta, and refused to leave the country when she had the opportunity, afraid she would not be allowed back.

It was not clear if the protests over power cuts would fizzle or blow up into a broader show of dissatisfaction.

Thein Aung Myint, 39, one of the organizers of the Mandalay protest, said the power supply had improved there on Wednesday night and residents in central Yangon said the same.

But activist Ko Htin Kyaw, 49, said there had still been power outages in the Yangon suburbs and he reserved judgment on the government's promise of increased power supplies.

"We have to wait and see whether they are as good as their word. This is not the first time they've said that," he said.

Kyaw Soe Lin, 45, a former political prisoner who has also been involved in organizing the protests, suggested his aims were broader.

"Personally I can't be satisfied with a boost in electricity supplies at the moment," he said.

He noted the government had blamed the power shortage in part on the attack by the Kachin rebels. That showed there was still strife in the country and the government needed to work towards sustainable peace.

The protesters accuse the former military government of enriching themselves at the public's expense by selling natural gas to neighboring China while Myanmar, among Asia's poorest nations, faces frequent power outages.

Power consumption in Myanmar, where only 25 percent of the population has access to the national grid, is one of the lowest in the world, averaging 104 kilowatts an hour per person, near the same level as the Democratic Republic of Congo and Nepal, according to the World Bank and Asian Development Bank.

Mountain lion killed in Santa Monica was probably seeking a home

In the end, the mountain lion was probably looking for a place to call his own.

Scientists believe the male mountain lion roamed his way down the Santa Monica Mountains early Tuesday, likely following a runoff channel. When daylight broke, he found himself in the middle of the city and scared.

The lion was 3, and experts said that was the age to carve out his own territory.

"These young guys are looking for a home of their own," said Jeff Sikich, a biologist with the National Park Service. "At this age, they are testing their boundaries and establishing their home range away from other males."

Officials on Wednesday were studying hair and tissue sample and preparing genetic analysis that could help shed more light on his origins as a debate raged over whether police should have killed the lion.

He and other experts believe the lion was startled as the morning commute began and noise of the city rose. Mountain lions ? more so than bears or other animals ? are harder to deal with when they enter civilization because they tend to turn aggressive.

"When you stick a dart in a mountain lion, it becomes agitated. A 90-pound mountain lion can easily take down a 200-pound deer," said Kevin Brennan, a wildlife biologist with the California Department of Fish and Game.

The mountain lion was one of at least 10 in the Santa Monica Mountains, Sikich said. Last year, another male mountain lion was killed when it tried to cross the 405 Freeway near the Getty Center.

But that case caused none of the furor of the shooting, which on Wednesday led to calls for independent investigations and policy changes.

Animal rights activists and others questioned how authorities were able to tranquilize and safely remove a 400-pound bear that had frequented Glendale last month, but not able to do the same for the feline.

"They could have done more to help this animal live," said Rachel Hoffman, 64, of Marina del Rey. "It's just a big cat."

The mountain lion was first seen shortly before 6 a.m. Tuesday walking along Arizona Avenue. A janitor spotted him in the U-shaped courtyard of an office building, pawing at a glass door. The animal tried to approach the janitor, but he ran and called police.

Authorities made several attempts to contain the mountain lion in the courtyard, said Andrew Hughan, a spokesman for Fish and Game.

Wildlife agents fired several tranquilizing darts at the animal as the animal moved in the small space. Only one dart pierced the cat, he said. The animal became aggressive and tried to flee the courtyard, which was near a preschool.

"The more agitated the animal is, the longer it takes for the immobilizing drug to take," Brennan said. "You have to note the behavior of the animal after 10 minutes" and possibly administer more drugs.

Officers used non-lethal bullets and firefighters employed a water hose to contain the lion while they waited for the drug to take effect. During that time, the animal got close to a Santa Monica police officer and it was fatally shot.

Police have said they killed the cat to protect the public, noting that the cat could have harmed children and shoppers in the bustling Santa Monica downtown district.

For a decade, the National Park Service has been monitoring the habits of the cougars living in the Santa Monica Mountains. Biologist are tracking five lions with GPS radio collars. The slain mountain lion was one of a handful not outfitted with a tracking device.

Mountain lions rarely wander into urban communities, scientists said. But the animals have less room to roam as development encroaches on the their land.

"To be a mountain lion is to roam, and you can't disperse across Southern California without running into a city like Santa Monica," Brennan said.
